Is Cushing’s syndrome responsible for cortisol face? Doctor busts the myths.
Recently, a topic has been trending continuously on TikTok – it is related to moon face. According to netizens, people with Cushing’s syndrome – who have high stress hormones – have puffy faces. This is called cortisol face – named after cortisol, the stress hormone. In an interview with HT Lifestyle, Dr Pranav Ghodi, senior endocrinologist at Wockhardt Hospitals Mumbai Central, said, “There have been a few posts on social media recently about ‘cortisol face’, which means puffiness or rounding of facial features. Social media influencers are blaming the stress hormone cortisol, but it is important to understand the facts behind this hormone.”

The endocrinologist further explained – “Cortisol is a steroid hormone produced by the adrenal glands (small triangular glands located above each kidney). It is often called the stress hormone, but it does much more than respond to stress – it is essential for the normal functioning of various internal organ systems such as maintaining blood pressure and blood sugar levels, regulating how our body uses carbohydrates, fats and proteins for energy, reducing inflammation, etc.”

High cortisol levels may be caused by:
- swelling, especially around the eyes
- flushing or redness in the face
- increased oil production in the skin
- loose or tired skin
Causes of cortisol facial:
Often social media influencers try to promote products that can cure cortisol face – this is not necessarily true. Lack of sleep, poor eating habits, excessive alcohol consumption, genetics and a person’s overall health can cause cortisol face or puffy face. “A rare case of too much cortisol (Cushing’s syndrome) due to a tumor in the pituitary gland or adrenal gland or excessive steroid medication from outside can cause facial changes. However, this happens after months to years of exposure and is accompanied by changes in other organs. Most people with cortisol do not have Cushing’s syndrome,” said Dr. Pranav Ghodi.
