Blocked fallopian tubes, also known as tubal occlusion, is a significant cause of infertility in women globally and in India. The fallopian tubes are essential for fertility because they allow the egg to travel from the ovary to the uterus, where fertilization occurs.

If they are blocked, sperm and egg cannot meet, preventing pregnancy. According to various studies, blocked fallopian tubes are responsible for 25-35% of female infertility cases in India.

A 2019 report by the Indian Society of Assisted Reproduction (ISAR) suggests that tubal infertility is one of the most common causes of female infertility in India. While male infertility and ovulation disorders also play an important role, tubal blockage remains a major challenge for women wishing to conceive.

A study conducted in Maharashtra, one of India’s largest states, found that 20–40% of women attending fertility clinics suffered from some form of tubal pathology, with blockage being the most common condition. Another study from Tamil Nadu revealed similar figures, with about 30% of women experiencing tubal infertility.

Due to blocked fallopian tubes

The main causes of blocked fallopian tubes in India are similar to global trends, but exacerbated by specific local factors –

1. Pelvic Inflammatory Disease (PID): In India, there is a high incidence of pelvic infections, often caused by sexually transmitted infections (STIs) such as chlamydia and gonorrhea, leading to tubal scarring and blockage. Poor hygiene practices and delayed medical intervention further increase the risk of developing PID.

2. Tuberculosis: A notable difference in India is the prevalence of genital tuberculosis, a disease that affects the reproductive organs and is a significant cause of tubal infertility. Studies show that about 10-15% of women suffering from blocked fallopian tubes in India are diagnosed with genital tuberculosis, which is much higher than in many western countries.

3. Endometriosis: Endometriosis is another common cause of blocked fallopian tubes. This is a condition where the tissue that normally surrounds the uterus grows outside of it, causing inflammation and sores in the fallopian tubes. Endometriosis affects about 10-15% of women in India, especially in urban areas where delayed pregnancies and changing lifestyles are becoming common.

4. Surgical History: A history of abdominal or pelvic surgery, such as for ectopic pregnancy or fibroids, may lead to adhesions or scar tissue, which can block the fallopian tubes. In India, unregulated and unsafe surgical procedures in some rural and low-resource settings further complicate the issue.

Diagnosis and treatment methods

In an interview with HT Lifestyle, Dr. Shobha Gupta, medical director, gynecologist and IVF specialist, Mother’s Lap IVF Center in New Delhi and Vrindavan, said, “Diagnosis of blocked fallopian tubes usually involves imaging tests like hysterosalpingography (HSG). Where there is dye involved. Injections are made into the uterus and fallopian tubes to detect blockages. Other diagnostic tools include laparoscopy and ultrasound, although HSG is most commonly used in India due to its cost-effectiveness.

According to him, the treatment options for blocked fallopian tubes depend on the severity and location of the blockage –

  • Tubal Surgery: In cases where the blockage is minor or near the end of the tube, surgical intervention to remove scar tissue or repair the tube may be effective.
  • In Vitro Fertilization (IVF): IVF bypasses the fallopian tubes completely by fertilizing the eggs outside the body and transferring the embryos directly into the uterus. In India, IVF is becoming an increasingly popular solution for women suffering from tubal infertility.

Chances of pregnancy if fallopian tubes are blocked

Pregnancy is possible after receiving therapy for blocked fallopian tubes. The type of treatment you receive and the degree of block will affect your chances of getting pregnant.

“If the blockage is close to the uterus the chances of a successful pregnancy increase. If there is a blockage at the end of the fallopian tube close to the ovary, the chances of success are reduced,” said Dr Shobha Gupta.

After surgery for damaged tubes due to an infection or ectopic pregnancy, your chances of getting pregnant are very low. Depending on how much and how much of the tube needs to be removed, the answer will vary.

“Before starting therapy, discuss your chances of a healthy pregnancy with your doctor. Blocked fallopian tubes are a major cause of infertility in Indian women, contributing significantly to the overall infertility burden in the country. The solution to this problem requires better awareness about reproductive health, timely medical intervention and access to advanced reproductive treatments such as IVF. “Early diagnosis and treatment, especially for infections and conditions like tuberculosis, are key to improving reproductive outcomes for women in India” concluded Dr. Shobha Gupta.
