When menopause comes, it can feel as if your whole life is thrown out of balance. Hot flashes, sleepless nights, mood swings and constant restlessness can be exhausting and deeply disruptive. Going through something so intense is challenging enough, yet many women are still silently expected to “tough it out.” But modern medicine offers a better way. Hormone replacement therapy (HRT) can significantly reduce symptoms and help women go through menopause with less discomfort.

Dr. Anshuman Kaushal, robotic, gastrointestinal, bariatric and laparoscopic surgeon and obesity specialist with over 27 years of experience at Apollo New Delhi, explains how hormone replacement therapy can support a spontaneous recovery. Menopause and why it is an essential measure for Indian women. In an Instagram video posted on November 14, the surgeon explains the benefits of HRT, addresses common misconceptions and outlines important tests needed before starting treatment.

Menopause needs to be destroyed

Dr Kaushal emphasizes that, due to limited awareness, many Indians fail to recognize the true severity of menopause symptoms. He explains, “Menopause is not drama; it is biology. And HRT is not a villain; it is science. In India, we treat menopause as if it is a woman’s personal ordeal that she has to endure silently. Menopause hits like an earthquake. Estrogen, progesterone, testosterone – everything drops. Hot flashes, insomnia, mood swings, weight gain, joint pain, Dryness, irritability, zero libido – all these things come and attack together.”

How does HRT help?

The surgeon explains, “HRT, or hormone replacement therapy, replaces what the body has stopped making. Modern HRT is safe, effective, and life-changing when started at the right time. It improves sleep, mood, bones, heart health, and even sexual health.” They outline the different ways in which HRT can help with menopause symptoms and benefit your health.

  1. HRT reduces hot flushes by about 75 to 90 percent.
  2. Improves sleep – Works better than melatonin gummies.
  3. Estrogen stabilizes serotonin, which improves mood and anxiety.
  4. Estrogen and testosterone can improve vaginal dryness and libido, helping to re-establish a well-functioning hormonal ecosystem.
  5. The risk of osteoporosis is reduced by 30 to 50 percent, thereby restoring bone and joint health.
  6. Opting for HRT before age 60 or within 10 years of menopause may improve heart health.
  7. Collagen production increases, skin hydration improves and a healthy glow is restored.

Does HRT cause cancer?

Dr Kaushal says the biggest fear associated with HRT is the perception that it causes cancer. However, he dismissed this, saying that anyone who still thinks so is relying on old data from the early 2000s. The surgeon says, “Modern guidelines such as NAMS, ACOG, or NICE guidelines suggest that when therapy is initiated at the right dose, at the right time, the benefits outweigh the risks for healthy women.,

He outlines safer ways to administer HRT – such as transdermal estradiol (gel or patch) and micronized progesterone – noting that these are far superior to older synthetic pills.

Who should avoid HRT?

Hormone replacement therapy is generally safe for most people, except those who have specific medical conditions as directed by the surgeon. He highlights, “HRT cannot be given to everyone. Those who have active breast cancer, clotting history, liver disease, uncontrolled BP, or unexplained bleeding. But a gynecologist will evaluate these thoroughly.”

Indian women need HRT even more

Dr Kaushal says Indian women have early menopause due to various health factors – low vitamin D, high insulin resistance, excess visceral fat, higher rates of bone loss. This makes HRT especially relevant and necessary for Indian women.

Before starting the therapy, Dr Kaushal says a series of tests should be conducted to ensure that your vital organs are fine and you are medically fit for the treatment. These include:

  1. thyroid profile
  2. fasting glucose
  3. insulin
  4. lipid profile
  5. vitamin D
  6. vitamin b12
  7. liver test
  8. pelvic ultrasound
  9. mammogram
  10. Consult gynecologist.

The surgeon concluded, “Menopause is not the end of life. But enduring it in silence can certainly feel like it. HRT gives women their lives back when nothing else is working.”
