Are You Targeting the Wrong Cholesterol Target? From diet to medications, here’s how young adults can protect their hearts

High cholesterol is a health concern that often goes unnoticed due to the lack of symptoms, causing it to silently increase the risk of heart disease and stroke. Low-density lipoprotein cholesterol (LDLc), also known as ‘bad cholesterol’, contributes to the formation of plaque in the arteries and causes damage, making it a ‘silent killer’.

Additionally, misconceptions related to the management of high LDLc levels impact treatment adherence and overall patient outcomes. Maintaining LDLC levels requires a lifelong commitment to maintaining target levels.

However, a growing trend suggests that people are discontinuing the medication due to the belief that once LDLC targets are met, continued treatment is no longer necessary. This is evident from a study published in Population Medicine, which found that about 40% of older adults with high cholesterol do not follow the prescribed regimen.

The silent killer hidden in your arteries:

This non-adherence can lead to progression of atherosclerosis, often canceling out any progress achieved during the medication cycle. In an interview with HT Lifestyle, Dr Viveka Kumar, Vice President and Head of Cath Labs at Pan Max – Cardiac Sciences in Delhi, shared, “Management of LDLC is not about achieving a one-time goal, it is about continuous, lifelong Maintenance: High LDLC levels may not cause any noticeable symptoms, but over time, this silent accumulation of plaque in the arteries leads to heart failure. Can lead to serious cardiovascular events such as seizures and stroke, even in patients who feel completely healthy.

He revealed, “In my experience, I have seen 40% of people who could have treated this condition but missed that opportunity because of misconceptions. Therefore, it is essential for patients to understand that LDLC management is an accurate, evidence-based approach to reducing cardiovascular risk. Every patient has unique risk factors, so treatment plans must be personalized to meet those needs. The LDLC target level is often considered a fixed figure, when in reality, it varies depending on individual risk factors, family history, and existing health conditions. Recent CSI guidelines2 emphasize the importance of individualized LDLC targets tailored to each patient’s risk profile. For example, individuals with diabetes and hypertension may have more stringent target LDLC levels (for example, <70 mg/dL) than those with significant comorbidities.''

High Cholesterol Myths Busted:

There is a common misconception among people that when treating LDLC, lifestyle changes such as improved diet and increased physical activity are enough to effectively manage LDLC goals, making medications seem unnecessary or unnecessary. Dr. Viveka Kumar highlights, “Lifestyle modifications play an essential role in supporting heart health, but they often fail to effectively control LDLc levels in the individual, especially in high-risk In persons. Clinical evidence underlines the importance of combining lifestyle adjustments with pharmacological treatments to achieve optimal LDLC reduction.

Another myth is that elevated LDLC is only relevant to older adults, further undermining early preventive care. Dr. Viveka Kumar said, “Elevated LDLC, with its associated cardiovascular risks, can affect individuals of all ages, including young adults. According to CSI Guideline 2, it is recommended that young adults should undergo lipid profile assessment to early identify and manage dyslipidemia, especially if they have a family history of cardiovascular disease or a genetic predisposition. Early identification and management of elevated LDLc can help prevent the development of coronary artery disease (CAD) later in life, reducing the likelihood of heart disease in adulthood.

Changes in treatment paradigms also reflect evolving standards in LDLC management. Dr. Viveka Kumar said, “Historically, cholesterol-lowering drugs were primarily prescribed to patients with LDLC levels above 130 mg/dL for people with diabetes or 160 mg/dL for non-diabetics. were determined. This changed when new draft guidelines for the UK’s National Health Service (NHS) suggested widespread use of cholesterol-lowering drugs, recommending that they reduce heart attacks or strokes by less than 10 percent within the next decade. May also be prescribed for at-risk individuals, but older protocols regarding use of the drug are still more widely followed.

He added, “Concerns about side effects from cholesterol-lowering medications, most commonly muscle pain, are another barrier to adherence. Although medications can cause side effects such as muscle pain, these adverse reactions are not universal. According to CSI guidelines, many patients tolerate cholesterol-lowering medications well, and those who experience side effects may benefit from dosage adjustments, alternative prescriptions, or complementary therapies. Awareness of potential side effects allows physicians to address patients’ concerns while ensuring effective LDLC management.

Addressing common misconceptions and consulting with your doctor for the most appropriate treatment for your diagnosis can support more effective treatment outcomes and long-term cardiovascular risk reduction.
